-Statement of Application Agreement
I understand that in order to be eligible for in-state tuition rates, the burden of proof lies with me, the student. Appropriate
documentation is attached in support of my request for eligibility for resident tuition rates (i.e. copy of driver's license). I agree to
notify the Registrar if there are any changes in the information submitted with this form. I understand that an out-of-state student
cannot attain resident student status simply by attending school/college for 12 months within the State of Alabama. I certify that if I
am granted conditional admission to the College, I will submit all required admissions records to the Registrar's office prior to the
issuance of first semester grades. Otherwise, my continued enrollment will be denied pending receipt of all admissions records. I
further certify that I have read and understand the College's admissions policies in the current JDCC catalog. I understand that if a
third party agency (state, federal, or private) that is funding my scholarship has not paid the College by the end of the registration
period or by a thirty (30) calendar day extension, I will be responsible for my payment of tuition and fees immediately. I further
understand that if payment is not rendered immediately, the College may administratively withdraw me from classes for which I
have registered. I further hereby state that all information contained herein is true and accurate to the best of my knowledge and I
understand that any falsification of said information may subject me to dismissal from the College or to such other disciplinary action
as may be appropriate to or called for by existing federal, state, and/or local statutes or by the college officials acting in accordance
with published institutional rules and guidelines.
I understand that Jefferson Davis Community College owns all photographs of students and participants taken at JDCC
events, and that JDCC reserves the right to use the photographs for college promotional materials, both digital and print. If I do not
