Requesting a Transcript

Fees...

There is no charge for an official transcript.

In person...

Complete and sign a request form in the Student Affairs office on the Brewton campus, or with the receptionist in the Administration Building on the Atmore campus 7:30am to 4:30pm, Monday - Friday (excluding college holidays).  For your convenience, you may print a Transcript Request Form from the file at the bottom of this page.  Then, you may bring the completed form with you when you visit either the Brewton or Atmore campus; or, you may mail or fax the form to the Student Affairs office (address and fax number available on the form).

By mail...

Requests may be submitted by letter and should include the following information:

  1. Student's legal signature
  2. Student's full name/name used while in attendance
  3. Current address
  4. Daytime telephone number
  5. Student ID number
  6. Graduation date and/or years attended
  7. Campus attended (Atmore, Brewton, Fountain, Holman)
  8. Name and address of the party to receive the transcript
  9. Date

By fax...

Please fax your request to (251) 809-1593 and include the following information:

  1. Student's legal signature
  2. Student's full name/name used while in attendance
  3. Current address
  4. Daytime telephone number
  5. Student ID number
  6. Graduation date and/or years attended
  7. Campus attended (Atmore, Brewton, Fountain, Holman)
  8. Name and address for the party to receive the transcript

Online...

At the main page of the JDCC website, click on the Hawk's Nest link.  At the login screen, you must log in to the website using your User ID and unique PIN. Once you have successfully logged in, click on Transcript Request under the Student Information Center and follow the directions to submit a transcript request online.  Note:  If you attended JDCC prior to 1990, you may not have access to the online student information system and must therefore submit your transcript request by mail or fax.

By telephone or e-mail...

Since we must have the student's legal signature before we are allowed by law to issue a transcript, requests are not accepted by telephone or e-mail.

Official transcripts...

Official transcripts are normally mailed directly to the receiving party.  Official transcripts in a sealed envelope may be picked up by the student in the Student Affairs office on the Brewton campus. The student must produce a picture ID in order to obtain the transcript. It will be up to the receiving party to accept the transcript as official or not.

Unofficial transcripts...

Transcripts issued directly to the student and/or faxed transcripts are unofficial transcripts. Unofficial transcripts may be picked up on the Brewton campus or the Atmore campus. Students requesting an unofficial transcript in person must produce a picture ID.

Additional information...

Transcripts are issued within three to five business days upon receipt of the written request from the student except during registration and drop/add periods.  There may be a delay during these times.  Students are responsible for requesting his/her transcript in a timely manner (i.e. two weeks before it is needed).

Students must show a picture ID when requesting and picking up transcripts in person. All obligations to Jefferson Davis Community College must be cleared before official transcripts can be released.  Transcripts may not be picked up by another party (i.e. parent, friend) unless the student has provided written authorization to do so in the request.  The designated person must produce a picture id before obtaining the transcript.  Transcript requests are to be signed by the individual whose name is on the transcript.
